I agree that my child may participate in the JJ Pearce Mustang Stampede Kids' Clinic. In consideration of participating in this event, I agree, on behalf of the above-named child to release, discharge, indemnify, and hold harmless RISD, its agents, servants, and employees from any and all claims, demands, damages, rights of action or causes of action, present and future, whether the same be known, anticipated, unanticipated, resulting from or arising out of participation in this event. I HEREBY GRANT PERMISSION TO THE JJ PEARCE STAFF TO OBTAIN MEDICAL CARE FROM A LICENSED PHYSICIAN, HOSPITAL, OR MEDICAL CLINIC FOR THE PARTICIPANT NAMED HEREIN AT SUCH TIME AS EITHER PARENT OR LEGAL GUARDIAN CANNOT BE CONTACTED. I give permission for the J.J. Pearce Mustang Stampede Program, to publish images of my child, for purposes of marketing the J.J. Pearce Mustang Stampede Program, and any events associated with the program. This activity is not sponsored or endorsed by the Richardson Independent School District.
By selecting "Yes," I, the parent/guardian of the student named above, agree to release to the District the student's name, voice, verbal statements, portraits (video or still) and consent to their use by the District. As well as provide the Pacesetter organization use of my student's photo on their social media.
